  • In the event of your child, named above, needing immediate medical attention at the hospital, the school will try to contact you. If we are unable to do so, please type your name below giving permission for your child to be accompanied by an adult from the school.

    The data requested will be stored on the school management information system and used for the purposes outlined in our Privacy Notice and in accordance with our Data Protection Policy. Also refer to the Welsh Assembly Government Privacy Notice issued in August 2017 which sets out what the Welsh Government does with the education related information that it receives from Schools and/or Local Authorities about Children and Young People.

    As outlined in the Privacy Notice, during your child's time at Corpus Christi there may be occasions when he or she will be in photographs or video recording celebrating his/her achievement or promoting the school. Typically, such photographs or recordings may be used, for example on the school website, Twitter or in our Prospectus.

    • Ensure that your child is valued for who he/she is helped to make good progress in their spiritual, moral, emotional, physical and academic development

    • Demonstrate our faith and our school’s foundation in the teachings of Jesus Christ, by what we teach and the way we live and worship in our school

    • Do our utmost to provide the best possible education we can for you child and enthusiastic teaching rooted in our beliefs, our values and our skills

    • Provide you with information about your child’s progress

    • Contact you if there is a significant problem with your child’s attendance or punctuality

    • Inform you of any concerns regarding your child’s behaviour, work, health or safety

    • Challenge your child to strive for the highest standard of personal, social and intellectual development and aim for excellence in all he or she does.

    I acknowledge the different and unique talents which God has given me and my responsibility to use them wisely.

    Therefore I will try to:

    • Attend school regularly and on time
    • Wear school uniform and bring all the equipment I need every day
    • Observe the school rules, inside and outside of the classroom as listed in my school diary
    • Treat everyone with the respect they deserve
    • Learn something new each lesson and always do my best
    • Complete all my homework and classwork on time
    • Try to think for myself and take responsibility for my actions
    • Follow teachers’ instructions and behave sensibly  so we can be happy and safe as we learn
    • Take care of all school equipment and property and keep Corpus Christi free from litter and chewing gum
    • Share my feelings honestly and politely and show consideration for others in school
    • Act responsibly on school buses and set a good example when on school trips or when representing the school
    • Involve myself in the life of the school
    • Tell an adult if anyone is being bullied

  • Parental Responsibilities

    We acknowledge that we as parents are the primary educators of our children and have an irreplaceable role to play in supporting our children’s learning at Corpus Christi

    Therefore I / we will try to:

    • Ensure that my child attends school regularly and on time: suitably equipped and in full uniform
    • Inform the school of any concerns or problems that might affect my child’s work or behaviour
    • Support the Christian values of Corpus Christi
    • Emphasise to my child the importance of keeping the school rules which are listed in my child’s school diary
    • Give my child the opportunities for home learning and support the homework given by the school
    • Read and sign my child’s school diary every week
    • Support the school’s behaviour police (including Positive Discipline) by praising my child when they receive stickers and rewards and by supporting the school when sanctions such as after-school detention are necessary
    • Attend parents evenings and discussions about my child’s progress
    • Encourage my child to be enthusiastic about learning and to enjoy school
    • Encourage my child to show kindness and consideration to others
    • Talk to my child about their experiences in school and encourage them to do their best

    Support the Catholic community and the school governors in their responsibilities for maintaining the school buildings in good repair.

  • Parent /carer consent for routine off-site visits 2023 - 2027 (or end of Yr 11)

    I hereby agree to my child participating in routine visits off the school/centre site. These visits might include the following, or similar, activities:

    Sporting fixtures, Theatre Visits, Geography Field Trips, Retreats, Exhibition Visits, Young Enterprise Events, Music Events, Activity Days and other one day Educational Visits.

    I understand that:              

    • such visits will normally take place within one day. They will often be outside normal school hours but adequate advance notice will be given so that I may make appropriate arrangements for my child’s return home;
    • my specific permission will be sought for any visits beyond those listed above or which could involve commitment to extended journeys, adventurous activities or expense;
    • all reasonable care will be taken of my child during the visit;
    • my child will be under an obligation to obey all directions given and to observe all rules and regulations governing the visit and will be subject to all normal school/centre discipline procedures during the visit;
    • I agree to pay for any loss, damage or injury which may be caused to any person or property solely through the misconduct or carelessness of my child;
    • I will repay the expenses of any such members of the party reasonably caused by any accident to/illness of my child, or as a result of any need to accompany him/her home following serious misconduct;
    • I must inform the school/centre of any medical or psychological condition or physical disabilities that may affect them during the visit; and update during the year (or subsequent years) if necessary.
    • I consent to the giving of any urgent medical or surgical treatment to my child which is considered necessary by the medical authorities during the school visit.
